Job Description:
Full-time Nurse Practitioner position welcoming new grads,
focused on home risk assessments in Salem, VA.
Conduct comprehensive assessments of adult and geriatric patients
in home, SNF, and community settings.
Review health history, medications, social factors, and identify
care gaps during visits up to one hour.
Collaborate with physicians, social workers, and caregivers to
develop care plans and provide follow-up.
Work Monday to Friday, 8 am to 5:30 pm, with flexible hours and
travel up to 15%.
Offer competitive salary ($102,300-$125,900), benefits, mileage
reimbursement, and a $7,500 sign-on bonus.
Require Master--------s degree or equivalent, current RN and NP
licensure, certification, and at least 1 year NP experience.
Must have valid driver--------s license, be willing to travel, and
possess strong communication skills.
