Job Description:
This RN Case Manager - Hospice role involves providing
compassionate, quality care to patients in their homes and
healthcare facilities, ensuring compliance with regulations.
Responsibilities include ongoing patient assessment, care planning,
symptom management, and caregiver education, with 60% of focus on
care delivery. Additionally, 30% involves maintaining accurate

Qualifications include an active Ohio RN license, at least one year
of acute care experience, proficiency with EMR and Microsoft
Office, and a valid driver--------s license. Work requires
standing, walking, lifting up to 50 pounds, and availability for
weekends, evenings, holidays, and on-call emergencies. Benefits
