Home Care Licensed Practical Nurse (LPN): Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The LPN would provide care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety. Hours: Full-time | Days | Primary hours would be Monday - Friday from 8:00am - 5:00pm | Weekend and Holiday Rotation is dependent upon staffing levels. Work approximately two Holiday's per year and one weekend every 4 - 6 weeks. A few must-haves for this position: - Current license as an Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing. - 2 years experience as an LPN. - Able to travel within geographic area serviced by the office. Some functions of what you would do as an LPN: - Participate in ongoing interdisciplinary assessment of the patient. - Provides services that are ordered by the physician as indicated in the plan of care. - Recognizes and reports changes in the patient’s condition to the RN. - Counsels and educates the patient and family in meeting healthcare related needs. - Completes timely and accurate clinical notes including addressing patient’s progress.
